Ladies, our monthly prayer meeting is this Saturday at Hillcrest park in Fullerton, 9-11 AM. Come pray with us and then join us for a little group fitness lead by Lisa after our prayer time. Methylene blue (ProvayBlue) is an intravenous medication and dye best known for treating methemoglobinemia but has been found to have potential benefits in other areas of medicine ranging from the treatment of ifosfamide neurotoxicity in oncology to the staining of various ducts/tracts (ureters, breast ducts, etc.) to identify them during procedures. 🏥💉 . Methemoglobin (metHb) contains oxidized iron in the ferric form (Fe 3+) which has higher affinity for oxygen compared to normal deoxyhemoglobin with iron in the ferrous form (Fe 2+). This limits metHb's ability to offload oxygen at the tissue level. Methylene blue treats metHb by reducing the iron moiety back to the ferrous state via an NADPH-dependent reduction through methemoglobin reductase. As an intensivist, I’ve used methylene blue a handful of times for this indication. It’s important to know that at higher doses, methylene blue can actually CAUSE methemoglobinemia by converting ferrous iron to ferric iron. 👨🏽⚕️🤓 . As an anesthesiologist, I use methylene blue for refractory vasoplegia coming off cardiopulmonary bypass. In this context, methylene blue inhibits nitric oxide synthase and guanylate cyclase to decrease the amount of cGMP and, in turn, decrease vascular smooth muscle relaxation. This should (hopefully) increase systemic vascular resistance! Given the paucity of evidence, if I’m thinking about administering methylene blue, it’s always a “last resort” after trying pressors like vasopressin and norepinephrine. That being said, at least one randomized trial showed a benefit in reducing mortality in patients with vasoplegia following cardiac surgery. 😷❤️ . Remember that methylene blue is also a monoamine oxidase inhibitor (MAOI) and should therefore be used cautiously in patients already on MAOIs and/or SSRIs. Furthermore, patients with G6PD deficiency should not receive methylene blue due to the potential hemolytic crisis that can ensue. ☠️ . Tag a friend
I’ve seen some ugly things in church. I have. And I still go. I go because I want to be the change and the church I want to see. I go because someone will be there who needs me. I go to serve. I go to contribute my drip to the anointing that is there to change lives and break chains. I go because the faith of others strengthens mine. I go for fellowship. I go so that I can be equipped to change the world around me on Monday. ON SUNDAYS, I GO TO CHURCH because that broken, hurt by their last church, seeking salvation, on the edge person may come through the door and I’ll have a hug for them that will break pain and bondage off of their lives. So, on Sundays... I go to church.
